Couples Therapy Agreement
This agreement for couples therapy is between {name-1} and {name-2} and {select-1} and the basis for which we intend to conduct the relationship are set out by the conditions below.
Counselling sessions
Each counselling session will last 60 minutes or 90 minutes depending on what is appropriate. The whole period of each session is your time. If you arrive late I will still be available for you, however the session will still end at the originally agreed time.
Fees
The cost of each 60 minute session will be £70.00. This is payable at the start of each session. If there are any changes to the fees there will be a minimum of 6 months notice prior to this which I will provide notice of both verbally and in writing.
Cancellation
If you are unable to make an appointment, please contact me to change the day, the time, or cancel the appointment. A minimum of 7 days notice is required, otherwise 50% of the fee is payable. If you do not attend 3 consecutive sessions the counselling relationship will have to end.
Should one client not attend an appointment, I will not be able to hold the session without agreed consent of both parties.
Confidentiality
As a member of the British Association for Counselling and Psychotherapy (BACP), I adhere to their Code of Ethics.
The content of all counselling sessions is confidential and this applies to any and all records, in accordance with the Data Protection Act, except in the following instances:
- Where the client gives consent for confidence to be broken.
- Where the counsellor is compelled to break confidentiality by a court of law.
- Where information disclosed during a session is of such gravity that confidentiality cannot reasonably be expected to be maintained. This will apply, for example, in cases of fraud and crime and where there are reasonable grounds to believe that there is a risk of serious harm to the therapist, clients or others.
Our Relationship
This relationship remains within each session and any external contact in relation to appointments (phone, e-mail, or letter). If we should accidentally meet outside of a session, I will always acknowledge you in brief and friendly manner, unless you express your wish not to be acknowledged. This matter shall be discussed at the beginning of the relationship.
Medical History
If we decide to work together, you will be asked to provide some basic information about yourself including your G.P. contact details, any medication you are taking and history of emotional difficulties.
I agree to the terms of this contract which has been fully explained to me.
